The Grifone hit the woodwork early on with Giovanni Simeone, who is reported to be on the verge of a move to Fiorentina for €18m.
Mattia Perin needed a tough save on Eguelfi, but Cesena converted a penalty through Karim Laribi after Aleandro Rosi brought down Gliozzi.
Simeone got the equaliser straight after the restart with a right-foot strike that took a deflection to wrong-foot the goalkeeper.
Cesena were pinned back, but held out until forcing extra time at Marassi.
Diego Laxalt took just five minutes to give Genoa the lead for the first time, tapping in at the end of a well-worked team move down the right.
It was a bittersweet evening for the Grifone fans, as both Simeone and Laxalt are expected to leave next week.
