Alexandre Pato is out of tonight's showdown due to a flu virus, so Antonio Cassano, Kevin-Prince Boateng and Zlatan Ibrahimovic face Juventus.
Pato has reportedly gone back to Milan along with Massimo Oddo, as both are running a temperature.
The Brazilian was set to start, but has instead been replaced by Cassano to complete the trident with Boateng and former Juventus striker Ibrahimovic.
As Cassano is cup-tied, this formation rests Robinho for Wednesday's Champions League trip to Tottenham.
Max Allegri must do without Andrea Pirlo, Pippo Inzaghi, Massimo Ambrosini, Daniele Bonera, Luca Antonini and Alexander Merkel, so Marek Jankulovski is again used as left-back.
Juventus have Simone Pepe suspended, plus Fabio Grosso, Hasan Salihamidzic, Leandro Rinaudo, Paolo De Ceglie, Momo Sissoko and Fabio Quagliarella injured.
Alberto Aquilani has failed his fitness test on a muscular problem, so does not make the starting XI, moving Claudio Marchisio to central midfield.
Luca Toni and Alessandro Matri are again the front pairing, but the big change is Giorgio Chiellini returning to centre-back after a few weeks on the left.
Juventus: Buffon; Sorensen, Chiellini, Barzagli, Traoré; Martinez, Marchisio, Felipe Melo, Krasic; Toni, Matri
Milan: Abbiati; Abate, Nesta, Thiago Silva, Jankulovski; Gattuso, Van Bommel, Flamini; Boateng; Cassano, Ibrahimovic
