The fixture ended 1-1 after 90 minutes, as Xian Emmers pressured goalkeeper Daniel Grimshaw into an error and pounced to open the scoring after 12 minutes.
It was played at the Academy Stadium in Manchester and the hosts equalised through a penalty awarded for a debatable Davide Bettella challenge on Lukas Nmecha, who converted it himself.
Jens Odgaard was booked in the second half for simulation when he went down in the box, but Inter boss Stefano Vecchi called for a penalty.
It was all square after 90 minutes and went straight to a penalty shoot-out.
That started badly for Inter, as Facundo Colidio’s first attempt was saved, but Vladan Dekic made up for it by parrying both Matthew Smith and Phil Foden strikes.
Matteo Rover’s effort was also kept out by Grimshaw, but Felix Nmecha had the chance to win it and blasted over the bar.
Nicolo’ Zaniolo could win it, but again, his penalty was saved by Grimshaw and it went to sudden death.
Brahim Diaz’s attempt was saved, Manuel Lombardoni hit the woodwork and finally Bettella’s penalty was parried by Grimshaw.
