According to Tuttomercatoweb and Voetbalnieuws, the winger is set to sign on until June 2020.
The 28-year-old had been the subject of offers from Inter, Liverpool and Arsenal in recent months.
Mertens joined Napoli from PSV Eindhoven in July 2013 for around €9.5m.
His current contract holds him to the Stadio San Paolo until June 2018.
