El Shaarawy had gone to Monaco on loan last summer, but moved over to Roma in January and rediscovered his form with seven goals in 15 Serie A appearances.
Roma have an option to buy for €13m, but La Nazione newspaper believes they could solve two problems in one fell swoop.
The Giallorossi see Dzeko being worth around €13m and would offer him to Milan in a straight swap for El Shaarawy.
The Bosnia international struggled in his debut Serie A season, missing numerous open goals and netting eight in 30 games.
Roma will have to pay Manchester City another €11m in July on top of the €4m already handed over for the loan with mandatory option to buy.
Dzeko has been regularly dropped to the bench in recent outings, but Milan could do with a centre-forward if Carlos Bacca walks away and Mario Balotelli returns to Liverpool.
